Ani Mells, a spirited girl from River Province, is ambushed by Governor Felling’s wardens during a trip to the marketplace with her best friend, Weevil. Her world changes, and she fears she will not see her family or Weevil again.
On Attic Island, Ani sees the bleak environment and the despair of its residents. Governor Felling’s officials, including Matron and Warden Silas, control the island. Ani is assigned a bunk in a crowded dormitory and learns the strict rules and small food portions. She sees many people suffering but also notices that some, like her, seem healthy despite their 'diagnosis.' Her questions are met with threats. She befriends a boy named Dell, who is truly sick, and starts to notice odd things about how the Scourge is managed and how the 'infected' are treated. This makes her suspect something is wrong.

They plan their escape, knowing they must get past the island’s defenses, the wardens, and the actual Scourge that affects some prisoners, including Weevil, whose health worsens.

These people reveal they have been working on a real cure for the Scourge, different from Felling’s fake version. They explain that Felling has suppressed this cure and used false diagnoses to keep political control and exploit River Province resources. They offer to help Ani and her friends but warn that the cure is unstable and needs precise use.
With the hidden community's help, Ani and her allies plan to broadcast the truth about Felling’s conspiracy and the real Scourge cure to River Province. They find an old radio tower on the island. Weevil, very ill but determined, helps Ani, despite his weakening state. As they try to activate the broadcast, Matron and her wardens confront them. Weevil creates a diversion, drawing the wardens away from Ani, giving her time to transmit the message. He collapses soon after, his fate uncertain, but his bravery allows Ani to expose the governor’s crimes.
