In a post-apocalyptic world ravaged by four devastating alien waves, a lone teenage girl must decide whether to trust a mysterious stranger to save her brother, or risk losing everything to the insidious fifth wave.
The story opens with Cassie Sullivan, a typical high school girl, thinking about the world before the Others—aliens—arrived. A large alien spaceship, the Mothership, appears in Earth's orbit. This begins the First Wave: an electromagnetic pulse (EMP) that disables all technology, plunging the planet into darkness. The Second Wave follows quickly, causing huge tsunamis that destroy coastal cities and kill billions. The Third Wave releases a deadly avian flu virus, killing most of the remaining population. Cassie, her younger brother Sam, and their father survive the initial attacks, living in makeshift camps with other refugees. Life is a constant struggle for survival with few resources and growing paranoia.
As the survivors gather in refugee camps, the Fourth Wave begins. Military Humvees, led by Colonel Vosch, arrive at Cassie's camp. They promise to take children to safety at a facility called 'Wonderland.' Cassie's father is among the adults who are later killed. This is part of the Fourth Wave's plan to eliminate adults. Cassie's younger brother, Sam, is taken by the soldiers. Cassie tries to follow him onto a bus, but she is accidentally left behind and sees the mass execution of the remaining adults. Alone and desperate, Cassie promises Sam she will find him. This starts her solitary and dangerous journey.
Cassie travels alone through a destroyed Ohio, driven by her promise to Sam. She lives by a strict rule: trust no one, as the Fourth Wave has blurred the lines between human and alien. After weeks of scavenging and avoiding the Others, a Silencer, an alien posing as a human, shoots her in the leg. Near death, Evan Walker finds and rescues her. He is a seemingly kind and resourceful young man living alone in a secluded farmhouse. Cassie, suspicious of everyone, at first resists his help, but her severe injury forces her to rely on him. She slowly heals and develops feelings for Evan, despite her deep mistrust and constant fear that he might be an Other.
Cassie's suspicions about Evan are confirmed when she discovers he is an alien. Evan confesses he is one of the Others, a Silencer, but claims to have left his kind because of his growing empathy for humanity, especially Cassie. He explains the true nature of the Fourth Wave: the soldiers are not human, but Others, using human children to create the Fifth Wave—child soldiers unknowingly implanted with alien consciousness. He reveals that Sam is likely at Wright-Patterson Air Force Base, where the children are being trained. Despite her shock and feeling of betrayal, Cassie, desperate to save Sam, agrees to work with Evan to infiltrate the base.
Meanwhile, the story shifts to Ben Parish, Cassie's former high school crush. He survived the initial waves and was later taken by the alien-controlled military. He is called 'Zombie' and is part of Squad 5301 at Wright-Patterson Air Force Base. Ben is trained to fight the Others, whom he believes are the 'infesting' aliens. He does not know that the soldiers training him are the true enemy. He undergoes physical and psychological conditioning, becoming a hardened soldier. He forms a bond with his squadmates, particularly a young girl named Ringer, and a quiet boy named Sammy, who is Cassie's brother, Sam.
Ben and his squad are sent on what they believe are missions to eliminate Others. During one mission, Ringer discovers a small, metallic implant, called a 'chip,' at the base of her skull. She realizes all the child soldiers have these implants, which control them. The 'Others' they are sent to kill are actually uninfected humans. The children themselves are the Fifth Wave—unwitting instruments of human extermination. The alien plan is to use humanity against itself, turning children into killers by making them believe they are fighting aliens. In fact, they are hunting down the last uninfected adults.
Cassie and Evan carry out their plan to infiltrate Wright-Patterson Air Force Base. Evan uses his alien abilities to create diversions and eliminate guards, clearing the way for Cassie. Cassie, armed and determined, navigates the complex, using her wits and training from Evan. She is driven only by the desire to find Sam and get him out alive. Evan, however, has a deeper, more dangerous plan to destroy the base's core infrastructure. Their journey through the base is full of tension, narrow escapes, and brutal encounters with the alien-controlled soldiers.
Inside the base, Ben, Ringer, and the rest of Squad 5301 are ordered to kill uninfected civilians. Ben, now fully aware of the deception thanks to Ringer, refuses. He starts a plan to save Sammy and escape. He finds Sammy, who is still a shell-shocked child, and tries to comfort him. Ben, Ringer, and a few other squad members fight their way through the base, clashing with the alien-controlled soldiers and the adult Others. Their escape is a desperate struggle for survival. The child soldiers use their training against their former captors.
As Cassie searches for Sam, she meets Ben and his squad. The reunion is tense and disbelieving, especially for Ben, who is shocked to see Cassie alive. Cassie is relieved to find Sam with Ben, but escaping is the immediate priority. They realize they are all fighting the same enemy, though their experiences of the 'waves' have been very different. They join forces, combining Cassie's determination and street smarts with Ben's military training and Ringer's tactical skills, to navigate the final, dangerous stages of their escape from the collapsing base.
Evan reveals his plan to destroy the base's power core, creating a large explosion that will disrupt the alien operations. He tells Cassie that he must stay behind to ensure its success, sacrificing himself. Cassie, Sam, Ben, Ringer, and the remaining squad members escape the base as Evan detonates the explosives, causing a massive chain reaction. The explosion cripples the alien command center but leaves the survivors with an uncertain future. They emerge into the destroyed landscape, now united and determined to fight back. They understand that the war for Earth is not over and the line between friend and foe is dangerously unclear.
